Responsible to: Practice Manager

Our Client is supporting practices across Rotherham to increase capacity.  One element of this is to increase clinicians that are available each day, the other side is the increase the number of Care Navigators.  Care Navigation underpins General Practice and ensures practices run as efficiently as possible.

Job Summary

Based within a Rotherham practice, the individual will join an existing Care Navigation Team.  The role will be primarily to take incoming calls from patients, assess their need and direct to the best possible resource for their individual need.  It will be a busy role that will make a huge difference to the practices we are supporting.

Key Responsibilities

  • Answering telephone calls
  • Navigating patients to the most appropriate resource
  • Booking appointments
  • Utilising clinical systems
  • Greet patients and visitors
  • Support any administrative duties as required by the practice
  • Facilitate effective communication from patients to the clinical team
  • Cancel clinics should this be required – calling patients where necessary
  • Filing and copying as required
  • Data entry as required
  • Adhere to patient confidentiality policies
  • Any other duties deemed necessary and appropriate


You will be dealing with patient’s confidential information and as such the Data Protection Act should be considered when dealing with sensitive information.

Person Specification


  • English & Maths “C” or above or equivalent
  • Technical Experience
  • Good IT Skills (MS Office)
  • Previous reception experience
  • Experience of EMIS / SystemOne
